The procedure, in the order it happens

IOL power is calculated from precise measurements of your eye's axial length and corneal curvature using non-contact optical biometry (IOL Master or equivalent).
- 2Pre-op counselling
You review the IOL options (monofocal, aspheric, extended, toric, EDOF, trifocal) with your surgeon and choose based on your lifestyle, glasses tolerance and budget.
- 3Same-day admission (Day of surgery)
Arrive ~90 minutes before the scheduled surgery. Fasting is NOT required for topical-anaesthesia cataract surgery. Dilating drops are instilled and the eye is prepared.
- 4The surgical procedure (typically 12–15 minutes)
Under topical anaesthesia (drops only, no injection), a 2.2 mm micro-incision is made. Phacoemulsification uses ultrasound to break up the natural lens; the pieces are aspirated; the chosen IOL is folded and inserted. Duration varies with case complexity.

What happens Day 1 to Week 6
Home with a soft pad on the operated eye. Mild watering / gritty feeling is normal. Sleep on the un-operated side.
First post-op review in clinic. Pad removed. Vision usually functional; you start the eye-drop schedule.
Vision improves progressively. Avoid dust, direct water on the eye, and heavy lifting. Sunglasses outdoors.
Second follow-up. Most patients return to office work; drivers wait until Day 10-14 for full clearance.
Vision stabilises. Spectacle prescription (if needed for reading or fine astigmatism) finalised at 4-6 weeks.
Things patients often get wrong
Isn't cataract surgery only for very old people?+
No. Cataracts can start in the 40s in people with diabetes, high myopia, chronic steroid use or previous eye trauma. The right time to operate is when the cataract starts interfering with what you want to do — driving at night, reading, work — not a specific age.
Should I wait until the cataract is 'ripe'?+
No. Modern MICS phacoemulsification is easiest on a soft, early-to-mid cataract. Waiting until it's mature makes the surgery more complex and increases the risk of complications. Waiting is old advice from the pre-phaco era.
Will my new IOL wear out and need replacing?+
No. The IOL is designed to last a lifetime. What can happen 6-24 months post-op is thickening of the natural capsule that holds the IOL — this is fixed in 5 minutes with a YAG laser (not another surgery).
Honest counsel — when to pause, treat elsewhere, or defer
- Uncontrolled diabetes (HbA1c > 9) or uncontrolled hypertension — surgery is deferred until systemic control improves.
- Active eye infection or severe dry eye — treat first, operate later.
- Very early cataract that isn't affecting daily life — better to observe with 6-month reviews than to operate prematurely.
- Amblyopic (lazy) eye that hasn't developed useful vision since childhood — cataract surgery in this eye won't restore lost vision.

Ayushman Bharat (PM-JAY) covers cataract surgery for eligible beneficiaries with a basic foldable lens. Any premium IOL upgrade (aspheric, extended, toric, EDOF or trifocal) is billed as a separate patient-pay top-up on top of the scheme. Private cashless insurance covers most IOL tiers with a policy-tier co-pay. Biometry, medicines, OPD visits and YAG capsulotomy (if needed later) are billed separately.
